OPEN ENROLLMENT IN PROGRESS!

Open Enrollment will occur this year beginning Monday, May 18, 2015 until Friday, June 5th. Every benefits eligible employee needs to consider whether they want to make changes during this period.

YOU MUST COMPLETE AN ON-LINE ENROLLMENT FORM regardless of whether you are enrolling for the first time, declining coverage, making changes or keeping your current elections in place. Failure to complete this form will result in your benefits ending on June 30, 2015. Following is important information to review as you consider your options during this enrollment period:

IMPORTANT INFORMATION:
Despite some increasing costs, the employee portion for our medical and dental plans will be staying the same this year.  Your percentage of contribution toward medical coverage is based on your annual salary range. The employer/employee contributions chart for 2015/2016 is in the process of being edited to reflect a higher amount that Eden II will need to pay on your behalf, but you can use the chart to determine what your contributions will be (no changes from 2014/2015).

There are no planned changes this year to copays for visits or prescription drugs, and the improvements built into the plan last year continue (e.g., covered flu shots at pharmacies, a no-copay annual well-visit/physical).

There is no change in the rates for our Vision Plan with Davis Vision. A new lens option is now also available within the plan (the Ultra-Progressives available now with a $140 copay were previously not covered by the plan and can cost in excess of $300 without insurance).

While Eden II provides short term disability, long term disability, and life insurance to employees eligible for these benefits (see this website for more information), many employees chose to supplement their short term disability and life insurance on their own. We have a relationship with Allstate to allow eligible employees to purchase these benefits through payroll deduction. If you are interested in accessing additional short term disability or life insurance, please check off the box on the enrollment form and an Allstate representative will contact you (or, based on employee interest, may schedule a session in the near future at our sites).

HR Representatives will be visiting each site during the Open Enrollment period (May 18th- June 5th) to assist with any questions. The schedule will be posted on the website and will also be provided to our Directors to post at their locations.

For those employees who do not have their own computer either at work or home, please ask your supervisor to help you access one at your job site.

Annual open enrollment is a good time each year to review your beneficiary designations for Life Insurance and your pension to ensure they reflect your current choices. If you need assistance in this area, please reach out to your HR representative.

This year, eligibility for medical plan participation is changing related to regulations in the Affordable Care Act. Any employee who averaged at least 30 hours of work each week between Nov, 2014 and April, 2015 may be eligible for participation in the plan as of July 1, 2015 (even if they are not a regular “full time” employee). HR is reviewing employee worked hours and will reach out to employees who may be eligible. (Those employees will need to enroll for, or decline benefits, through this open enrollment process).

Our prescription drug plan is staying the same as far as drugs covered/co-pays, but the provider is changing to a different provider through MagnacareRx. This provider is based in our area, and we are hopeful will provide improved service. Please continue to use your current card for prescription drugs when needed, but make sure that as of July 1 you present your new card to your pharmacist to avoid any problems. 

It is critical that this information is shared with ALL BENEFIT ELIGIBLE EMPLOYEES. As you know, many of our employees DO NOT have e-mail and we are asking for the Directors help at each location to ensure that copies are provided for each and every employee at their locations. We don't want anyone to lose out on their benefits because they were not informed.
